My dad found this while going through his stuff. I completely forgot about this thing, it brings back some memories. Maybe it will for you to? If not it was a liability card for skating the park. Don't remember what park it was I don't think it was called bones brigade headquarters, possibly powell skatepark somewhere in LA. It was in a lot videos back in the day it had the big skateboard statue from the animal chin video.
